What we do
The Provincial Home Dementia Care Program is led by skilled nurse practitioners who provide personalized care and support for individuals living with moderate to advanced dementia in the comfort of their own home. A nurse practitioner will assess and manage your care needs, offering guidance on symptom management, medication, and overall well-being. A physician and other health-care professionals can provide additional support and expertise as needed, ensuring comprehensive care for your loved one. This collaborative approach helps ensure the highest level of attention and tailored care for each patient.
Who we see
People who have a confirmed diagnosis of moderate to advanced dementia, who have a functional impairment, and are living at home can avail of this service.
How to access this service
The HDCP accepts referrals from:
- Primary Care Providers
- Community Support staff
- ER physicians
- Acute care physicians and social workers
- Specialists such as a Geriatrician
Self-referrals are not accepted; however, your health-care providers can make a referral by contacting the Home Dementia Care Program located in your zone. There are no costs to participate in this program.
Our Team
The Home Dementia Care Program health-care team is comprised of trained professionals who specialize in the care of the elderly and may include a nurse practitioner, an occupational therapist, a social worker, and a geriatrician or care of the elderly physician
