Social Work Services (Acute Care)

Social Work is a profession concerned with helping individuals, families, groups and communities resolve problems that affect their well-being. Social Workers in hospitals assist patients and their families to cope with problems that accompany illness. These problems can make hospitalization more difficult and interfere with recovery.

Social Workers provide support in coping with the stress that can go with a difficult diagnosis or treatment. They also provide counseling and guidance by talking over problems and helping find solutions. They can help with communication among patients, family and members of the health care team. They can also help to locate and arrange access to supportive services in the community such as home supports, personal care homes, long term care placement, rehabilitation as well as mental health and addictions services to name a few.

Social Workers are available to assist patients on the medicine, surgery, dialysis, intensive care, palliative care, rehabilitation, obstetrics and pediatric units as well as the Emergency Department.

Social Assistance Workers provide services that involve financial matters that may arise during a patient's hospital stay including: home oxygen, medical equipment and supplies, transfers to other hospitals, home supports, long term care, personal care homes and medical discharge costs.

Social Assistance Workers help patients and families determine if they are eligible for government subsidies and programs while ensuring the client's information is treated in the strictest confidence.
