Home Nursing care is a nursing procedure completed by an experienced Registered Nurse who will evaluate your needs in an ongoing basis and adjust services as necessary. These nurses work closely with your physician and other health care staff to provide high quality care to assist you in achieving the highest level of independence in your care.
Short term Acute Care
For select services Western Health will provide nursing care up to 14 days duration.
This care will replace hospital admission or facilitate early discharge from Inpatient care. Referral must be completed by the attending physician. All supplies, including medications will be provided at no cost to the client to treat admitting condition.
Acute Care
For nursing care up to two months duration Western Health will provide a maximum of one nursing visit per day and will provide the necessary supplies to complete the procedure.
Note: Medications will not be provided. You must get a prescription from the doctor for all medical supplies.
Chronic/Rehabilitation
For nursing care requiring longer than two months, nursing visits will continue up to a maximum of once a day. Supplies to complete the procedure will not be provided and other means of obtaining the necessary supplies will have to be explored.
