Adult Day Support Program

The Adult Day Support Program is a program of planned daily activities designed for the individual who is physically and/or socially isolated.

Service Provision

The program offers nutritious meals, professional counseling, monthly blood pressure screening, tub baths, as well as a variety of leisure type activities. Rehabilitation is another major component of the service. Through guidance, stimulation, and referral to available services, the program strives to maximize the individual’s potential for independent living.

All of the clients are encouraged to take in any of the available social/recreational/spiritual activities of the main facility. Transportation, via a wheelchair accessible bus, will be provided within a limited geographic radius to those who have no alternate means.

Who Can Apply?

Referrals are accepted from any individual, family member or agency and all admissions must be voluntary. An assessment will be completed on all individuals requiring this service.

Program Attendance

Clients are encouraged to attend the program for as long as it continues to meet their need, or that of their care provider. Participation in activities provided is strongly encouraged. If it appears that the program is no longer meeting an individual’s need, or the care of that client has increased beyond our resources, it may be necessary to re-evaluate their continuance in the program.
